Campus Health Check Announcement
To avoid overcrowding, the health check will be conducted in staggered sessions by department. Students are requested to attend the checkup according to the designated time slot for their department.
Date: Saturday, September 20, 2025
Check-in Location:
NB201 Classroom, B2, Gymnasium (Check-in)
MN Building, B2 Atrium and Classrooms (Examination)
Fee: NT$500 (payable on-site)
Students from low-income households must present valid official documentation for verification on the day of the checkup. Only government-certified documents will be accepted; documents issued by neighborhood chiefs will not be recognized. No supplementary documents will be accepted after check-in. Eligible students can apply for a free health check at the “Payment Counter” on-site.
Students who complete the checkup at an external medical facility are not eligible for fee waivers.

Note:
- Lunch Break (11:30 – 13:00): No health checks will be conducted during this time.
- To reduce congestion, students are encouraged to attend according to their assigned department time slot. If unable to do so, students may choose any available time slot instead.
- Doctoral, Master’s, Continuing Education, In-Service Master's Programs, transfer students, and undergraduate students entering graduate programs may choose any of the above time slots.

Reminder:
- Prior to the checkup, please visit the hospital website and complete the online health survey
(Available after September 8). The QR code generated after completing the questionnaire is required for check-in. - Fasting is not required for this health check.
Off-Campus Health Check Instructions
Students unable to participate in the on-campus health check on September 20, 2025 may instead complete a health examination at:
- Our partner hospital – Country Hospital, or
- Any other hospital (excluding diagnostic clinics), using Shih Chien University's Health Check Form.
Important Guidelines
- If you choose a non-partner hospital, please ensure that the examination includes all required items listed on the back of the university’s health check form.
Note: Labor, food service, military service, and residence application health checks (for overseas Chinese or foreign students) do not meet Ministry of Education requirements. - Missing items must be supplemented. The final results must be transcribed onto the university form, officially stamped by the medical institution, and submitted in person or mailed to the Health and Counseling Section, 1F, Building G.
- Health check period: From June to September 2025.
If your examination already meets the university’s requirements, please submit the original copy by September 19, 2025. Late submissions will not be accepted.

For Transfer Students
Transfer students may submit a copy of their freshman health check report (within the past 2 years) from their previous university.

- Height, weight, waist circumference, blood pressure, pulse
- Vision, color vision, hearing
- Head & neck, chest & general appearance, abdomen, spine & limbs, skin
- Oral and dental check
Blood Test
- CBC: Hemoglobin (Hb), White Blood Cell count (WBC), Red Blood Cell count (RBC), Platelets, MCV, HCT
- Blood Lipids: Total Cholesterol
- Kidney Function: Creatinine, Uric Acid (UA), BUN
- Liver Function: SGOT(AST), SGPT(ALT)
- Hepatitis B: HBsAg, Anti-HBs
Urinalysis
- Protein, Sugar, Occult Blood, pH
Chest X-Ray – Standard full chest X-ray
Partner Hospital Information
Special Discount Period:
September 22, 2025 (Monday) – October 31, 2025 (Friday)
Outside this period, the fee will return to NT$1,000.
