Dear New Students,

According to the Ministry of Education's School Health Act, all new students must complete a health examination.

1. Transfer students (who may submit a previous health examination report from their former school) and students who have not yet completed the health examination from the previous semester must ensure that they complete the examination and submit the report to the Health Center by April 1.
2. Transfer students or those who have not yet completed the health examination in the previous semester should visit a medical institution or Country Hospital to complete the examination.

• Application Period: February 3, 2025 – March 31, 2025
• Discounted Fee: NT$550
• Examination Hours:
  • Monday to Friday: 08:00–11:00 AM, 13:00–15:00 PM
  • Saturday: 08:00–11:00 AM (excluding national holidays and the Lunar New Year holiday)
• Check-in Location:
  • No. 61, Sec. 4, Ren'ai Rd., Da’an Dist., Taipei City (106台北市大安區仁愛路四段61號)
  • 1st floor – Take a health check-up number ticket at the health check-up counter and wait for your turn.

Important Reminders

1. Please bring the following items:
   • Face mask
   • National Health Insurance card or passport
   • QR code from the completed questionnaire
2. After the examination, the hospital will notify the school of the health report. Students can also check their reports online and do not need to submit a hard copy to the school.
3. If you have a fever or feel unwell, please wait until your symptoms have subsided before going for the examination.
